Here are Chicago NA Region forms and documents you can view or print.
| The CRSC official policy document, which was approved in April 2005 meeting, has been amended several times since then.. The official policy was last amended in September 2009. Click on the link to view or print your own copy. | |
| CRC Policy | As of July 2010 the Chicagoland Regional Convention Committee policy document is currently in development and will be posted when it becomes available. |

| Financial Policies and Operating Procedures | These policies and procedures apply to the Service Office (CSO), Regional Convention (CRC), and the Regional Service Committee (CRSC). This document was approved by the CRSC in Apr 2009 and the CSO-BOD in October 2009 |
| Insurance Policy FAQs (Frequently Asked Questions) | The CSO maintains an insurance policy that offers general liability coverage to the NA groups and Area Service Committees (ASC) within the region as a whole. This document seeks to answer several frequently asked questions about the insurance coverage |
| CRSC Policy (Word format) | This Word format version of the CRSC policy is password protected and is used by the Policy Committee in order to make updates when the policy is amended. |
| CRSC Policy (Open Source format) | This Open Source format version of the CRSC policy is password protected and is used by the Policy Committee in order to make updates when the policy is amended. |
| Financial Policies and Operating Procedures | This Word format version of the Financial Policies and Procedures is password protected and used to make updates as needed. |
| Note: Some of these documents are password protected because they are intended only for addicts actively participating in our service structure. The password has been provided to all members of the CRSC. The RCM's have been asked to provide the password to their areas. The password is freely available to all addicts in the region who request it. |
| Monthly Area Report Form | Regional Committee Members (RCM's) representing Service Areas use this form to make their monthly reports at CRSC meetings |
| Group Update Form | GSR's and other group trusted servants, RCM's, and other concerned addicts use this form to submit adds, changes, and deletions to the Region's Meeting Directory |
| Regional Service Profile | Candidates for Region service positions describe their qualifications on this form |
| Topic for Discussion | Members at the monthly CRSC meetings use this form to bring up new business for consideration. |
| CRSC Expense Reimbursement Voucher | Members use this form for expense reimbursement. |
| CRSC Check Request Form | Members use this form when requesting a check. Please note that all contracts, invoices and other required backup documentation must be attached. |
